BAC Local Union 15 Welfare Fund Provider Directory

 

The Fund has negotiated special contracts with a network of area Physicians and Hospitals known as Preferred Health Professionals (PHP). These participating providers will render services for fees that, in most cases, are below prevailing prices. These providers are referred to as In-Network.

 

 

You may contact PHP to determine whether or not your provider participates in any of the following Networks.

You can contact PHP on line at www.phpkc.com and begin a search to find a provider or look to see if your medical provider is in-network.

 

If you have trouble using the Web site or do not have access to the Internet, the Preferred Health Professionals Customer Service Department is available to assist you in your provider search. They are available Monday through Friday from 8:00 a.m. – 4:30 p.m. CST. Their phone number is 913-685-6300 or 800-544-3014.

 

Dental and Vision services are paid at the same rate so it is not necessary to locate an In-Network Provider.

Prescription Drug Expense

 

Your Prescription drug benefit is provided by Express Scripts. When you are eligible for benefits, you will receive a Prescription Drug Card that can be used at most pharmacies. For further information, please refer to your Summary Plan Description or contact the Pharmacy Help Desk at 800-235-4357.
